Patient: Jayden (pseudonym)
Age: 11 years old
Diagnosis: ADHD and ASD
Custody Case: Jayden, an 11-year-old Primary 5 student, initially performed well in lower primary but began experiencing academic challenges in upper primary. His parents and teachers observed that he had a short attention span, was easily distracted and tended to avoid tasks requiring sustained effort or multiple steps. Jayden also struggled with planning and organization. Social issues, which were present since preschool, became more pronounced in primary school, where he struggled to form friendships and interact with peers, despite being compliant and quick to grasp concepts. He often kept to himself and had no close friends.
Promises Services: Jayden’s parents were recommended to Promises for a psycho-educational assessment. During the initial consultation, the psychologist discussed their concerns and explained the assessment process, covering cognitive, achievement, social-emotional, and behavioural evaluations, as well as session details and expectations. Feedback was gathered from parents, teachers, and Jayden himself, ensuring a comprehensive and reliable evaluation of his challenges for a well-rounded understanding of his needs.
Patient Response: Through the comprehensive psycho-educational assessment, it was found that Jayden met the diagnostic criteria for having Attention Deficit Hyperactivity Disorder (predominantly inattentive subtype) and Autism Spectrum Disorder, level 1. The psychologist reviewed the findings with Jayden and his parents, helping them understand his strengths and challenges. Together, they discussed effective home strategies and school-based interventions to support Jayden’s needs. He continued therapy sessions with the psychologist, focused on improving social skills and executive functioning. Additionally, he was referred to see a Promises psychologist to explore the benefits of psychopharmacology for him. With approved access arrangements for examinations and strong home-school collaboration, Jayden made significant academic and social progress. Now in secondary school, he is thriving, showing continued improvement with psychiatric and psychological support, and appears happy and well-adjusted.
Patient: Jane and John (not their real name)
Age: Early 40s and mid 30s
Diagnosis: Violation of trust within the marriage and lack of connection and intimacy.
Custody Case: Jane and John have been married for about 10 years. Jane was involved in an emotional affair and shared nude pictures with someone of the opposite sex. As a result of this betrayal, John is considering divorce, and they sought therapy to process their hurt and identify the root causes of their issues.
Promises Services: Couples were assessed both together and individually to identify challenges related to emotional disconnection, trust violations, and a lack of communication and shared rituals for connection. They expressed confusion and distress about their circumstances and struggled to break free from a cycle of blame. During therapy, they learn to identify their needs and express them more effectively, while also understanding the warning signs of a breakdown in connection, and the triggers for their emotional outbursts
Patient Response: Both partners were able to open up to Winifred, building significant rapport and trust. They gained an understanding of how each had contributed to the breakdown of their marriage, as well as the gaps in their communication, conflict management, and emotional regulation skills. They learned more effective ways of communicating, which allowed them to share their deepest dreams.
Support Extended: Within eight sessions over a period of four months, they shifted their decision from wanting a divorce to investing in their marriage and finding a shared dream of getting a new home. This remarkable progress occurred as both partners sincerely worked to let go of past hurts through the therapeutic process, learning more effective ways to express their feelings and establish boundaries to safeguard their marriage.
Patient: Peter (Pseudonym)
Age: Early 50s
Diagnosis: Anxiety, Panic Disorder
Custody Case: Peter was an expatriate gentleman in his early 50s. He worked for a multi-national company in a senior position. After the COVID-19 restrictions were lifted and people started gradually returning to the office instead of work-from-home arrangements, he started experiencing work stress-induced symptoms including breathlessness, tightness in his chest, feeling flushed when having to present at meetings or giving a speech, and thoughts that he was going to embarrass himself in front of others. He realised that due to the company’s restructuring efforts during the COVID-19 period, the dynamics at work had changed and become more toxic. His boss had become more negative towards him and he no longer felt safe voicing his opinions for fear that he may be fired. He was also having to get used to in-person presentations again after a long period of doing Zoom meetings and presentations which seemed less threatening. Apart from work, he generally led a healthy lifestyle, running and swimming in his free time.
Promises Services: Peter sought treatment at Promises and worked with a therapist skilled in cognitive-behavioural therapy (CBT). The therapist guided him through breathwork and mindfulness-based exercises to help him reduce his baseline level of emotional and physiological distress. Then, using cognitive reframing, Peter was assisted to identify and challenge the dysfunctional beliefs he had about work, his boss, and public speaking. He was also guided by the therapist to learn how to manage his panic attacks through guided exposure exercises.
Patient Response: Within 5 - 8 sessions, Peter was feeling better and noticed that he was able to carry out the tasks that he used to fear at work. He was able to attend major stakeholder and networking events, talking to and giving speeches without having a panic attack. He said he could not believe that he had gone from avoiding work trips and anticipating the worst before meetings, to volunteering for chairing committees and setting up new initiatives at work. With help from his therapist, Peter re-evaluated his work goals and whether these were aligned with his values and priorities. As he grew more confident at work, he also felt more empowered to think about other job options and have conversations with his boss about reviewing the scope of his work, in line with his values of autonomy and creativity.
